Berlin Questionnaire
The Berlin Questionnaire is an important tool that helps healthcare professionals and physicians determine how sleep apnea is affecting your sleep and health. It's quick and easy, consisting of ten questions used to evaluate risk factors for sleep apnea, such as snoring behavior, sleepiness or fatigue, and obesity or hypertension. Epworth Sleepiness Scale (ESS)
This scale was also developed to determine how lack of sleep is affecting daily functions and if your level is drowsiness is serious enough to warrant a sleep study.

It is also important to note that if your spouse, partner, friend, or family member tells you that they think you stop breathing in your sleep or if your snoring is affecting their ability to sleep, you should seek your physician's help or call SleepHealth for more information.

Use the following scale to choose the most appropriate number for each situation:

0 = no chance of dozing
1 = slight chance of dozing
2 = moderate chance of dozing
3 = high chance of dozing

Sitting and reading
 
Watching TV
 
Sitting inactive in a public place (e.g., a theater or a meeting)
 
As a passenger in a car for an hour without a break
 
Lying down to rest in the afternoon when circumstances permit
 
Sitting and talking to someone
 
Sitting quietly after a lunch without alcohol
 
In a car, while stopped for a few minutes in traffic
 

1-6 - Congratulations, you are getting enough sleep!
7-8 - Your score is average
9 and up - Seek the advice of a sleep specialist without delay
